SPF 50 HIGHEST LABEL. SPF 50 is now the highest assigned SPF value permissible. There is insufficient evidence that higher SPF’s provide sufficient protection.

CANCER PROTECTION ONLY at SPF 15 of HIGHER. Products must be broad spectrum (both UVA-A & UV-B ray protection) & have an SPF of 15 or higher to claim they protect against skin cancer and premature aging. SPF lower than 15 can only say they protect against sunburn.

CAN’T SAY WATERPROOF Waterproof, sweatproof, and sunblock are prohibited terms. They cannot claim more than 2 hrs. of protection without the need to reapply. They can still say “water resistant” if they state how many minutes after sweating or swimming you will be required to reapply.

EMR: A Wedge between You & your Doctor

The electronic medical record (EMR) or the electronic health record (EHR) is quickly becoming an integral tool in the primary care and other office settings. While many support its use for efficiencies of information storage and information sharing, reduction in errors from illegible handwriting, and ease of third party payment, I believe the single greatest flaw that has resulted it use is the complete loss of eye contact with your health care provider.

My experience on the patient’s side of the desk has been doctors keeping their heads bent down to the screen in a busy effort to complete the necessary keystrokes and electronic checkmarks for the encounter. I can’t remember the last time a doc looked at me since these tablets and other devices have infiltrated the medical office setting.

What is being missed here is the grave importance of listening to the patient’s history in order to reach an accurate diagnosis. More than eighty percent of the information necessary to reach a diagnosis for treatment comes from the medical history. About another ten percent from the physical exam and the smallest percent come from diagnostic tests such as labwork and imaging. Sir William Osler said, “Listen to your patients. They’re trying to tell you what’s wrong.” How can a doctor listen adequately to a patient if they have their eyes glued to the screen of an EMR?

It seems to me, if a doctor cannot listen with eye contact, there cannot be accurate history taking. Without this vital information, there is a greater likelihood of hit-and-miss scenarios with repeat visits, redirecting treatment plans, or worse -- missed diagnoses leading to a more serious medical scenario. Where is the efficiency here?

This complex problem is one of the most challenging in cosmetic medicine. So many factors contribute to this embarrassing presentation and can make you look old, tired, or hung over. While environmental issues such as allergies, alcohol intake, hydration, extreme weight loss, rubbing your eyes, and insufficient sleep can contribute, this dilemma is primarily hereditary and worsens with aging.

The skin under the eyes is the thinnest on the body and thus allows the purple/red dark underlying tissues to be more visible. A tenancy toward even thinner more translucent skin can be inherited. As we age the skin gets thinner and thinner. Additionally, fat around the eye area atrophies with aging, further compounding the problem.

The single greatest defense in reversing dark circles is to fortify the skin with nourishing agents to thicken it, thus concealing the underlying darkness. An excellent product for this kind of nourishment to produce collagen and elastin is OBAGI's ELASTIDERM. Additonally, LASER TREATMENTS to encourage skin turnover and thickening are recommended.

Sometimes a vessel repair cream can help if broken capillaries are involved such as GLYMED's ROSACEA RELIEF. Bleaching agents do not offer relief as this is not a hyperpigmentation problem. Some experts recommend retinoid agents to encourage skin turnover; however, they are generally too irritating on this thin fragile skin.

5 Reasons to FADE your Tattoo Before doing a Tattoo Cover

1. The Tattoo Cover Art will not need to be as large. Frequently the artist must nearly double the size of the inked area in order to incorporate or integrate the old art into the new art.

2. The Tattoo cover will not need to be as dark. Artists typically lay down very dark and very heavy ink to provide adequate cover. Over time this heavy ink blurs and gets muddied up with the underlying ink leading to great customer dissatisfaction.

Before Getting a Tattoo

1. Reputable Facility. Get a referral from someone you know whose tattoo art you admire. Also, inquire as to the specific artist. Talent can vary greatly even at any given facility.

2. Basic Black. The easiest color for a laser to remove is black if you ever decide to get rid of your tattoo. The next easiest grouping of colors that respond quickly to laser removal are red, orange, and purple. Greens and blues, especially turquoise represent the greatest challenge to laser removal and can be quite costly to remove.

3. Size & Location. Keep it small to start, no larger than your fist. Avoid the hands and neck for anyone. And women report regretting the shoulders and ankles.

4. Choose Timeless Content. What’s in style this year may not be next year. The name or logo of a band or the name of a lover are often regretted.

5. Communicate. Clearly state your objectives to your artist and have them repeat it back to you to be sure they understand. Never say, “Use your judgment, whatever you think.”

Getting the Most from a Gym Personal Training Session: Part 2

If you are signing up for the first time or renewing a membership, try to negotiate one or more free sessions to be included in your package. Try talking to the membership folks to find out which trainer might be a good fit with you. Ask around and see if you can observe all or part of another member's session. This will give you an idea about the personality fit with the trainer. Again, as much exposure to the trainer you can get prior to committing to a package, the better. And I DO recommend committing to a package. You are more likely to get into routine and gain momentum if you enroll in a group of sessions (many trainers have minimum sessions anyway.

Getting the Most from a Gym Personal Training Session: Part 1 - VARIETY

Whether you are a new member or just needing motivation to get over a slump, keep the following in mind when you hire a personal trainer at your gym. In my opinion, eventual independence from the trainer should be a primary goal. It is not reasonable to have your workouts completely reliant on their presence as you are likely only going to be signed up for a limited number of sessions. With that in mind, the exercises should concentrate on those devices and machines that are available and do not need a spot/support person to accomplish. Strange and awkward balancing tricks with bands and balls are not practical as part of your long-term training regimen. You should not look like an audition-gone-wrong for Cirque du Soleil. Ideally, you will have them teach you about your body’s major muscle groups and demonstrate a variety of the options for working them (cable and pin machines, dumbbells, barbells, etc.). When the gym is overcrowded on a Monday night, you will be far more nimble to move about and find a free device (this will avoid discouragement during such busy times). Your diverse knowledge of the available options will keep your workouts interesting and avoid boredom and plateaus.

Medicis, the makers of Restylane and Dysport have gained FDA-approval for Restylane-L. Like Juvederm XC mentioned in a previous post, this is a hyaluronic acid pre-mixed with a Lidocaine numbing agent. I have not been detailed on this product yet and have not had the chance to use it, but I will let you know as soon as I gather knowledge and experience with it. I continue to have excellent feedback and results on bothe Juvederm XC and Dysport. Stay tuned!

Early Feedback on New Products: Dysport & Juvederm XC

So far, Dysport for me the injector, has been as easy to use as Botox with no detectable difference in my administration of the procedure (other than the 3:1 unit dosing conversion). My patients and myself can report less contact sting of the material at the time of injection with Dysport vs. Botox. Additionally, the onset of reduction of muscle movement appears faster (hours vs. 1 -2 days) especially in the challenging strong area between the brows (glabellar). As far as which seems to last longer..... it is too soon to know. Stay tuned. There is a slightly better pricing for Dysport supply from the manufacturer which I pass on to my clients in a lower list price.

Juvederm XC flows just as nicely and there is no difference from the injector's perspective on how it fills or corrects vs. Juvederm mixed manually with Lidocaine. The amount of pain perceived by patients appears to be even less than when mixed manually. One patient reported a lip fill as being completely painless. More to follow as we gain further experience with these 2 products.

I have been a member of a Health and Fitness Club of some kind all over the country for the past 35 years and have been a fan of weight training since Arnold first brought it mainstream. Recently I discovered greater Manchester's best kept secret little gym in Goffstown on Daniel Plummer Rd, near St. Anselm College. Gold's Gym of Goffstown is just celebrating their 2 year anniversary. This precious hideaway features a generous unshared parking lot, spacious clean and cozy locker rooms, a warm and friendly staff and all that you need in strength training, cardio equipment, and more. I couldn't help feel that old-school charm with the steel dumbbells. Yet, there are up-to-date machines of all kinds. The amount and variety of leg equipment was the greatest surprise. Freezing Away Fat

A recent study published in Lasers in Surgery & Medicine (Dec 09) by Avram and Harry describes a procedure known as Cryolipolysis. ZELTIQ™, a controlled cooling device, induces inflammation in fatty areas that result in the gradual reduction of fat over 2 - 4 months. The device applies suction on the outside of the skin and specifically cools the desired fat loss area, causing damage to only fat cells sparing the surrounding tissues. It is done as an outpatient, requires no pain management, and typically takes one hour. There have been no significant side effects identified as yet and there is no downtime. There is no evidence as of now that this device will play any role in reducing large areas of fat and is geared more towards normal weight individuals with modest fatty areas. Further study is needed to better understand the procedure. It is NOT FDA-approved for Fat Loss at the present time. We look forward to hearing more about this interesting procedure.

Laser Stretchmarks

This gentleman had bright red obvious stretchmarks from rapid muscular growth as a late teen/young adult about a decade ago. After 10 thirty-minute laser treatments over the course of one year, I was able to achieve a remarkable outcome. The stretched, "tissue-papery" thin skin shown in the before pictures was so transparent that the underlying red tissues were visible. The Q-switched Yag laser generated new healthy skin structure closing up the "window" to the redness below. This client was aware that there were no "sure-thing" cures for stretchmarks but was willing to patiently go through the process and take a chance, and he is quite pleased that he did.

"Drawing" Conclusions

My 10-week figure sketching course at NHIA is concluded. I am grateful for the experience and wish to thank all the models and my teacher Larry Christian. This subject of this final piece I’ve posted is actually a student from the class. His contemplative look gave me pause to review how this course has benefited my cosmetic practice. Over these months, I have honed my craftsmanship in liposculpting by gaining an appreciation for the flow lines of the human figure as they run along the entire length of the body. This conceptualization breaks the mold of the traditional medical approach to the body as isolated cosmetic units. Additionally, repeated drawing of lines of symmetry in the dynamic figure, particularly in torso twisted poses, accentuates my visual observation skills at detecting asymmetries so prevalent in all normal human anatomy. It is this keen observation that leads to greater accuracy of target areas enhancing the outcome of a liposuction case. I hope to have the opportunity to take “Sculpting the Figure” this spring at NHIA.

Well, I'm halfway through my 10 week course at the New Hampshire Institute of Art and I thought it was about time I shared one of my drawings. Normally, we only get one minute to capture the gist of the model (called gesture drawings). But we had 10 minutes to complete this one. I've definitely learned it's a lot harder than it looks to sketch from a live model! I highly recommend Larry Christian as a teacher for any future students.
